Saving information you have entered onto the performance evaluation forms accessed via the above links or from G:\UGALibs\PERS\Evaluation will result in that information being available for all Libraries' personnel to view. To avoid this, after choosing the form you need, save it on your local drive. Remember, leaving the form open without saving prevents others from accessing, so please save the forms you need as soon as possible.
